men and women, and the male reproductive system. This includes the kidneys, bladder, ureters,
urethra, prostate, testes, and penis.

You should consider seeing a urologist if you experience:

This depends on your insurance plan. Some plans require a referral from your primary care provider,
while others allow self-referrals. Please contact your insurance company to confirm.

Your first visit typically includes: 

 
Self-registration, you will receive a text message or email for you to register prior to your visit

● A review of your medical history

Discussion of your symptoms

● A physical exam (if needed)

● Possible lab tests or imaging studies
 
● Our providers aim to make your visit as comfortable and respectful as possible.
 

Our urology clinic does offer advanced treatments, including minimally invasive and robotic-assisted
procedures, which may result in less pain, shorter recovery time, and smaller incisions. Ask one of our
urologists when you see them in the office which options are appropriate for you.

Please bring: 

● A list of medications

● Insurance card and ID

● Relevant medical records or test results

● A list of questions or symptoms you’d like to discuss

You can schedule an appointment by requesting an appointment through our website, or by calling
our office.

We use Phreesia, a secure healthcare technology platform, to help patients confirm appointments and complete registration electronically.

You will receive email and text message reminders asking you to confirm your appointment and complete registration 5 days before, 2 days before, and on the day of your appointment. If your appointment has not been confirmed, you may also receive an automated phone call requesting confirmation.

To ensure you receive these notifications, it is important that we have your current mobile phone number and email address on file. 

Completing your registration and confirming your appointment in advance helps streamline your check-in process and may reduce your wait time on the day of your visit.

Yes. All patient information is kept strictly confidential in accordance with HIPAA regulations.

We now use FollowMyHealth as our online patient portal in place of MyChart. Through FollowMyHealth, you can continue to securely access and manage your health information, including viewing test results and visit summaries, messaging your providers and care teams, and review upcoming appointments.

Medical records requests can be submitted through our online medical records request service, which is available on our website under  Patient Resources. If you request medical records while in the office, you will be asked to complete this same online form.

Please submit your forms through our medical forms request process, which is available under
Patient Resources on our website. This ensures your request is received by the appropriate team
and processed as efficiently as possible.

Yes. We offer onsite laboratory services for biopsies, urine specimens, and blood samples. When applicable, your samples can be collected during your scheduled office appointment.

● Walk-In Blood Draws: Monday and Wednesday, 8:00 AM – 3:30 PM

Urine Specimens (Walk-In & In-Clinic): 8:00 AM – 4:00 PM
